COVID-19 Community Vaccination Clinics at KVMH and SMMH for
Pfizer Vaccine - First, Second and Third Doses
starts Wednesday, October 6, 2021.

KVMH and SMMH Vaccination Clinics now offers Pfizer boosters to ages 12 to 17 and must be at least 5 months out from their 2nd dose.
Walk-Ins Only – No appointments necessary

KVMH VACCINE CLINIC for PEDIATRIC AND ADULTS
Beginning in February, KVMH Vaccine Clinics will be open only in the afternoon due to the decrease in vaccination requests.
Wednesdays, 2:00pm – 4:30pm: Children ages 5-11 years old and Adults ages 12 and older
To expedite your visit, please print and complete two consent forms (STUDENT Vaccination Consent Form and KVMH Pediatric Vaccine Consent Form 11-2021) and bring with your child to clinic. Children must be accompanied by an adult.
We will accept consents signed by parents / guardians who cannot be present.

SMMH VACCINE CLINIC, in the Auditorium
Beginning in February, SMMH Vaccine Clinics will be open every other Thursday, starting on February 10th due to the decrease in vaccination requests.  There will be no vaccination clinic on Thursday, February 3rd.  Hours are from 2:00pm – 4:30pm

  • Thursday, February 10
  • Thursday, February 24

Offering Pfizer Vaccine – First, Second and Third Doses
To qualify for the Third Dose recipients must:

  1. Have already gotten two doses of Pfizer or Moderna vaccine at least six months ago
  2. Have a weakened immune system and at least 28 days have passed since second dose
  3. Have gotten Johnson & Johnson (J&J) vaccine at least two months ago

IN ADDITION, they must have one of the following:

  • Be 65 years of age or older
  • Live in a Nursing Home, Assisted Living or Foster Home
  • Be between 18 and 64 years of age with underlying medical conditions (Cancer, diabetes, asthma, HIV, heart disease, obesity) OR have work setting risk of COVID-19 exposure and transmission (workers in healthcare, fire, police, congregate care, education, daycare, food & agriculture, manufacturing, corrections, US Postal Service, public transit, grocery store)

YOU CAN RECEIVE A COVID-19 VACCINATION AT THESE LOCATIONS

PFIZER VACCINE CLINICS
Pfizer vaccine is available to those 12 years old and older. Minors’ ages 12 – 17 years old must be accompanied by a parent or guardian at their vaccination appointment.

 MODERNA VACCINE CLINICS
Moderna vaccine is available to everyone 18 years old and older.

JOHNSON & JOHNSON VACCINE CLINICS
The one-dose Johnson & Johnson vaccine is available to everyone 18 years old and older.
