PEDIATRIC VACCINATION CLINICS for Children Ages 5 – 11 years old

Adolescent and Adult doses will not be given at this time; Refer to Adult Clinic Hours
Clinics to be held at KVMH ONLY in front of the Medical Office Building on the following dates:

To expedite your visit, please print and complete two consent forms (STUDENT Vaccination Consent Form and KVMH Pediatric Vaccine Consent Form 11-2021) and bring with your child to clinic. Children must be accompanied by an adult.
We will accept consents signed by parents / guardians who cannot be present.

Offering Pfizer Vaccine – First, Second and Third Doses

Walk-Ins Only – No appointments necessary
Hours: 9:00am – 12:00pm (Noon)

To qualify for the Third Dose recipients must:

  1. Have already gotten two doses of Pfizer or Moderna vaccine at least six months ago
  2. Have a weakened immune system and at least 28 days have passed since second dose
  3. Have gotten Johnson & Johnson (J&J) vaccine at least two months ago

IN ADDITION, they must have one of the following:

  • Be 65 years of age or older
  • Live in a Nursing Home, Assisted Living or Foster Home
  • Be between 18 and 64 years of age with underlying medical conditions (Cancer, diabetes, asthma, HIV, heart disease, obesity) OR have work setting risk of COVID-19 exposure and transmission (workers in healthcare, fire, police, congregate care, education, daycare, food & agriculture, manufacturing, corrections, US Postal Service, public transit, grocery store)

KAUAI MOBILE VACCINE CLINIC

The Kauaʻi Mobile Vaccine Clinics are open for walk-in vaccinations for individuals ages 12 and older for first and second doses. The Comirnaty (formerly Pfizer) vaccine (2 doses for ages 12+), Moderna vaccine (2 doses for ages 18+) and Johnson & Johnson vaccine (1 dose for ages 18+) will all be offered at no cost.
Third doses for immunocompromised individuals will also be available.

The Kauai Mobile Vaccine Clinic will be closed on the following weekdays:

  • Thanksgiving: November 25 – November 27
  • Christmas: December 23 – December 25
  • New Year: December 30 – January 1, 2022

No appointments are needed, it is walk-in only. Vaccine recipients must be at least 12 years old. Those under the age of 18 need to be accompanied by a parent or a guardian, who will need to sign a consent form. Please bring a picture I.D. and an insurance card, if you have one.
The mobile vaccine clinic is a collaboration between the County of Kaua‘i, Hawai‘i Health Systems Corporation and Wilcox Medical Center.

PFIZER VACCINE CLINICS
Pfizer vaccine is available to those 12 years old and older. Minors’ ages 12 – 17 years old must be accompanied by a parent or guardian at their vaccination appointment.

 MODERNA VACCINE CLINICS
Moderna vaccine is available to everyone 18 years old and older.

JOHNSON & JOHNSON VACCINE CLINICS
The one-dose Johnson & Johnson vaccine is available to everyone 18 years old and older.
