POSITION TITLE: CLINICAL DOCUMENTATION SPECIALIST (OUTPATIENT) (REGIONAL)

STATUS: Exempt | Casual Hire (0.30 FTE)

LOCATIONS: This is a regional position that is located in the Health Information Management (HIM) department of the Hawai’i Health Systems Corporation (HHSC) Kaua’i Region, a public benefit corporation of the State of Hawai’i. HHSC Kaua’i Region consists of Kaua’i Veterans Memorial Hospital (KVMH) and Samuel Mahelona Memorial Hospital (SMMH), both critical access hospitals; The Kaua’i Region Clinics; and its affiliates.

The HIM department manages patient medical records, compiles essential statistics for cost analysis and service optimization, and oversees records management, including disease coding, birth documentation, and regulated record dissemination for ongoing care, insurance, and legal use.

JOB FUNCTIONS: The Clinical Documentation Specialist (Outpatient) (Regional) utilizes advanced coding and clinical expertise to direct efforts toward the improvement of clinical documentation through the roles of educator and consultant; facilitates improvement in the overall quality, completeness, and accuracy of medical record documentation through extensive record review; performs the daily review, analysis and coding of patient records; monitors coding for quality control; codes diagnosis and procedures; reviews and analyzes medical records; also trains new coders and any other duties as assigned.

This position has extensive interaction with physicians and other members of the healthcare team; achieves appropriate clinical documentation to support the medical necessity and level of services rendered to all patients; as well as participates in the development and delivery of education for providers and members of the healthcare team; identifies factors influencing and complexity and severity of patient diagnoses; work collaboratively with providers in the ambulatory setting to ensure overall specificity, accuracy, and completeness of clinical documentation to support the highest integrity of the medical record.

This position reports to the Regional Director of HIM, position no. 109770, or its designee.

MINIMUM QUALIFICATION REQUIREMENTS

To qualify, you must meet all of the following requirements.  Please note that unless specifically indicated, the required education and experience are credited based on a 40-hour workweek. All applicants are required to provide evidence of all applicable education, licenses, and certifications. 

Education: Bachelor’s degree from an accredited four (4) year college or university. Relevant substitutions allowed. Bachelor’s degree in Health Information Management preferred.

License: Current, valid type# 3 driver’s license preferred.

Certifications:

Required at the time of hire appointment:

  • Certified Coding Specialist (CCS) issued from The American Health Information Management Association (AHIMA); OR
  • Registered Health Information Technician (RHIT) issued from AHIMA; OR
  • Registered Health Information Administrator (RHIA) issued from AHIMA; OR
  • Certified Professional Coder (CPC) issued from The American Academy of Professional Coders (AAPC); OR
  • Certified Outpatient Coder (COC) issued from AAPC.

Present at the time of hire appointment or willing to acquire within three (3) years of employment:

  • Certified Documentation Integrity Practitioner (CDIP) issued from AHIMA or
  • CRT- Certified Documentation Expert Outpatient (CDEO) issued from AAPC or
  • Certified Documentation Expert Inpatient (CDEI) issued from AAPC or
  • CRT- Certified Clinical Documentation Specialist – Outpatient (CCDS-O) issued from The Association of Clinical Documentation or
  • Integrity Specialists (ACDIS)

Experience: Minimum three (3) years of professional coding work experience. Professional experience in training and auditing preferred.

Desirable Qualifications: Demonstrates dynamic personality, and comfort with public speaking and interacting with medical staff and administration. Must have strong public speaking skills, and motivation to promote the program. Possess the ability and willingness to accept autonomy with responsibility, and commitment to a collaborative practice.

Knowledge of: practices relating to quality care, utilization and reimbursement issues; practices in all clinical areas including medications and various lab and test results; understanding of insurance types; community resources; and basic computer skills.
