JOB LOCATION:  Kauai Veterans Memorial Hospital, Waimea, Kauai, Hawaii 

FUNCTION:  The Clerk IV is located in the Quality Improvement services of Kauai Veterans Memorial Hospital (KVMH).  KVMH is part of the Kauai Region, which includes Samuel Mahelona Memorial Hospital.  It is one of twelve facilities within Hawaii Health Systems Corporation.  The position requires travel, on a regular basis, to and from facilities within the region.

The primary purpose of this position is to provide clerical support services to the Quality Improvement department. There may also be frequent requirement for intense work activity and the ability to handle many clerical processes at one time while prioritizing functions.  Must have the ability to use tact and diplomacy in ensuring that HIPAA standards are maintained. High level of confidentiality required. 

Position may be subject to shift work and may be assigned to work in other facilities within the region. This position reports to the Regional Quality Director.

Requirements

MINIMUM QUALIFICATION REQUIREMENTS:

Education:  High school diploma or GED preferred.

General Clerical experience: Two and one-half (2-1/2) years of work experience which involved performance of a variety of clerical tasks (preferably clinical unit clerk experience) which demonstrated knowledge of English grammar, spelling, arithmetic, common office appliances and equipment; and the ability to read and understand oral and written instructions, carry out procedures in clerical work systems, speak and write simply and directly, compare words and numbers quickly and accurately, operate various kinds of office equipment.

Experience Preferred:  One (1) year of the above experience must be in a health care setting (e.g., hospital, clinic, physician’s office, etc) which involved working with physician(s) and receiving, coordinating, expediting and/or implementing their patient care orders.  Such experience must have demonstrated knowledge of medical terminology through the application of such knowledge to work assignments.  

Applicants for Clerk IV  must demonstrate possession of Supervisory Aptitude.  Supervisory aptitude is the demonstration of aptitude or potential for the performance of supervisory duties through successful completion of regular or special assignments which involve some supervisory responsibilities or aspects; by serving as a group or team leader, or in similar work in which opportunities for demonstrating supervisory capabilities exist; by completion of training courses in supervision accompanied by application of supervisory skills in work assignments; or by favorable appraisals by a supervisor indicating the possession of supervisory potential.  

Non-Qualifying Experience:   Experience limited to the performance of simple and repetitious clerical work which primarily involved a short cycle of tasks and manual dexterity shall not be considered qualifying.  Examples of such non-qualifying work include but is not limited to such tasks as wrapping and unwrapping articles; opening envelopes, folders and similar containers, affixing tags and labels; arranging, rearranging or tidying up office desks and equipment; delivering, distributing or collecting correspondence or parcels. 

Substitution of Education for General Clerical Experience: 

1. Successful completion of a substantially full-time equivalent clerical curriculum of a duration of a year or longer, leading to a degree or diploma at an accredited community college, or at a business or technical school which included courses in basic English, arithmetic, general clerical procedures, and office machines will be deemed to have met the experience requirements for the Clerk III. 

2.  Successful completion of a substantially full-time equivalent clerical curriculum leading to a degree, diploma, or a certificate of achievement at an accredited community college, business school or technical school which was for a period of less than one (1) year will be substituted for experience on a month-for-month basis provided the training included courses in basic English, arithmetic, general procedures, and office machines. 

3.   Partial Completion of Clerical Training: 

a.  Completion of half a school year of substantially full-time clerical curriculum at an accredited community college, or business and/or technical school which included courses in basic English, arithmetic, general clerical procedures, and office machines may be substituted for six (6) months of experience. 

b.  Completion of one (1) school year (a program of more than one year in length) of substantially full-time clerical curriculum at an accredited community college, or business and/or technical school which included courses in basic English, arithmetic, general clerical procedures, and office machines will be deemed to have met the requirements for the Clerk III.       

4. Education in an accredited university in a baccalaureate program with courses in English composition and college mathematics may be substituted for general clerical work experience on the basis of 15 semester hours for six (6) months of experience, up to a maximum of two (2) years.

FUNCTION: The Health Unit Clerk is part of the Psychiatric Unit of the Nursing Services Department of Samuel Mahelona Memorial Hospital (SMMH). The Psychiatric Unit at SMMH provides nursing, social work, and ancillary health services to involuntary and voluntary patients with acute and/or chronic psychiatric and behavioral problems.

FUNCTION:  This position is located in the Quality Department at Kauai Veteran’s Memorial Hospital (KVMH) but serves as a regional position providing services to KVMH; Samuel Mahelona Memorial Hospital (SMMH) and the West Kauai Medical Center (WKMC) which are a part of the Kauai Region, Hawaii Health Systems Corporation (HHSC), a public benefit corporation of the State of Hawaii operating 12 facilities statewide.

The Director of Quality serves as the facilitator responsible for the hospital-wide and clinic quality assurance (QA) and performance improvement programs in both hospitals and the clinics.  The incumbent plans, develops implements, administers, and manages a comprehensive program to improve patient care services, including patient safety, clinical outcomes, organization effectiveness and process efficiency.  This position ensures that QA and PI programs meet accreditation, licensing and other regulatory requirements, and takes the lead on Joint Commission readiness efforts.

The function of Director of Quality utilizes his/her knowledge and expertise to direct and oversee operations of Quality Management in both hospitals and the clinics, and works with infection control, utilization management, risk management, corporate compliance, leadership, safety officer,  and clinical departments in improving hospital and clinic processes.  This includes planning, coordinating, directing, organizing, and evaluating clinical outcomes and compliance with established Joint Commission, FDA, EPA, OSHA, CDC, and DOH standards.  This position is responsible for short and long range planning and promotion of interdisciplinary team practices.  This position requires travel, on a regular basis, to and from different facilities within the region as assigned and directed.

Major Duties and Responsibilities:

Electronic Medical Record Duties 70%

1. Responsible for analyzing all EMR Psychiatric and Emergency Room charts. Ensuring chief complaint, diagnostic impression, orders are completed.

2. Assign deficiencies to the appropriate physicians for chart completion and assist Physician in completing deficiencies with the EDM system.

3. Responsible for scanning all forms that require patient signatures, (i.e., downtime forms, ekg, ultrasounds, istats, rhythm strip, urine dip results) into the EMD software program for Emergency Room, Psychiatric and Long Term Care departments.

4. Process ROI request according to the HHSC policies and procedures and enter in data into the 3M tracking system.

5. Enter data into the Department of Health EDRS system for all Death Certificates.

6. Responsible for merging of duplicate files in Sorian Financials program.

7. Responsible for verifying patients who have trouble logging onto the Patient Portal.

8. Responsible for scanning of Long Term Care forms (i.e., Dental, post, progress notes, medical orders, Pharmerica, etc.).

9. Attend Meaningful Use meetings, and communicate information to HIM staff. 10. Obtain training for coding.

B. Clerical Duties 25%

FUNCTION: The Health Information Management Department (formerly Medical Records) is responsible for the hospital clinical information system, which is the basis for payment under prospective pricing and (DRG) diagnosis related groups. Adequate payment depends on accurate abstracting and coding and hospital’s cash flow depends on timely completion of all medical record functions.

The Health Information Technician III (HIM Operations Working Supervisor) supervises staff in the Health Information Management Operations Section and is responsible for maintaining the HIM department records and files; ensuring that patient medical records are completed timely; and processing requests for release of information. Other duties include scheduling of work assignments, training new employees, and other duties as assigned.

Major Duties and Responsibilities:

A. Supervision & Management of Section 45%

1. Plans, organizes and directs the daily operations of the HIM Operations section of the department:

a. Assists in the recruitment of staff.

b. Provides or oversees the orientation and training of personnel.

c. Reviews the work of subordinates for completeness, correctness and adherence to applicable policies and procedures.

d. In collaboration with the supervisor, completes annual performance evaluation of staff in a timely manner, holds subordinates accountable for their position responsibilities.

e. Provides supervisor with evaluation of staff performance to be used in the appraisal process, promotions and for disciplinary action.

f. Identifies employee problems and provides recommendations for resolution whenever possible.

B. Chart Completion 25%

1. Ensures that patient medical records are completed in a timely manner in accordance with the Kauai Veterans Memorial Hospital’s medical staff bylaws and rules and regulations based on Joint Commission (JC) standards and state and federal regulatory agencies, as well as to facilitate the billing process for Medicare/Medicaid and other third party payors.

2. Follows a system to review all medical records of patient admissions and subsequent discharges, monitor completeness of the medical records, notify physicians of outstanding charts, and initiate steps for disciplinary action, if necessary, for delinquent physicians, in accordance with the medical staff bylaws and rules and regulations.

3. Reviews the system on an ongoing basis to streamline procedures make adjustments to accommodate increase in medical staff, and bylaws and rules and regulations amendments, etc.

4. Performs complex quantitative chart analyses for discrepancies and for correction and/or completion by physicians and nurses, etc.

5. Codes diseases, operations, and special therapies according to the International Classification of Diseases.

6. Inputs clinical data, which provides indexes of diseases, operations, physicians and consultants according to, established plans, and procedures.

C. Records and File Management 20%

1. Maintains the HIM department files and ensures that the various outpatient records for the Emergency Room, Oncology, Endoscopy, Angiography, Cardiology, Ambulatory Surgery, Observation Unit, as well as inpatient acute care records, and other reports as required by licensing and regulatory agencies are filed accurately and in a timely manner. 2. Retrieves records to include:

a. Working with physicians, nursing and other hospital staff to retrieve past records, delinquent records, and incomplete records.

b. Locates “lost” records using lost chart format.

c. Retrieves records from off-site storage system

3. Receives, sorts, compiles loose records/reports received on a daily basis from various treatment areas (i.e., lab slips, radiology reports, ER reports, etc.) and files into the appropriate treatment (patient) records.

4. Assists in archiving medical records on an annually basis. Works with offsite facility to maintain accuracy and confidentiality of reports. Maintains work records to account for each record that is archived and retrieved.

5. Merges patient medical record numbers following policy and procedures assuring records are combined appropriately and accurately.

6. Coordinates all corrections with Patient Access Department regarding changes, which include inpatient status to outpatient, dates of treatment, multiple medical record numbers, etc. from prescribed hospital policy relating to conversions.

D. Release of Information 5%

1. Receives and processes requests from physicians, patients, and in-house requests for release of medical records information. Ensures that the appropriate signed authorization for release of information form is received prior to the release of records information and filed appropriately. Follows policies and procedures in the release of medical records information in accordance with state and federal regulatory agencies’ regulations.

2. Duplicates copies of medical records information as requested. Records release must be timely and strict confidentiality maintained. Maintains a log of the release of medical records information.

3. Receives and processes complaints in a timely manner.

JOB STATUS:  Permanent/Full-Time

SALARY: $6,989/month (SR20)

JOB LOCATION:  Kauai Veterans Memorial Hospital, Waimea, Kauai, Hawaii

FUNCTION:  This position is located in the Waimea Obstetrics Department, Kauai Veterans Memorial Hospital (KVMH).  The Obstetrics/Nursery unit provides all phases of nursing care involving prenatal care, disorders of pregnancy, complications of pregnancy, labor delivery, and care of the newborn and the mother.  This position is responsible for the provision of professional nursing care to patients in accordance with accepted standards and hospital policy.

The primary function of this position provides acute nursiing care and treatment services oon a 24-hour basis and is responsible for the total age-specific nursing care of the newborn, adult/adolescent patient in the unit.  In the event there are no patients in the unit, this position may be assigned to various units within the Nursing Services and may require cross training and floating as assigned.  Position is also subject to a combination of 12 and 8-hour shifts.

Requirements

MINIMUM QUALIFICATION REQUIREMENTS:

To qualify, you must meet all of the following requirements. Please note that unless specifically indicated, the required education and experience may not be gained concurrently. In addition, qualifying work experiences are credited based on a 40-hour workweek.

Education Requirements:  Graduation from an accredited school of nursing.

Experience Requirement: Six (6) months of responsible professional nursing experience.

Selective Certification Requirements: Applicants shall be restricted to those possessing the following:

  1. Of the required six (6) months of professional nursing experience, six (6) months of professional nursing experience working in an obstetrics department is required.
  2. Applicants are required to provide current certification in Basic Life Support (BLS) in Health Care Provider (adult, child and infant), NRP (Neonatal Resuscitation Program) and STABLE (Sugar, Temperature, Airway, Blood Pressure, Labs and Emotional Support) within one (1) month of employment.

Substitution: Possession of a master’s degree from an accredited college or university school of nursing may be substituted for the six (6) months of required Experience.

License Required:  License to practice as professional nurse in the State of Hawaii. Applicants must submit evidence of current Hawaii RN licensure (a copy of valid license) at the time of appointment.

KNOWLEDGE AND ABILITY REQUIRED:

KNOWLEDGE OF: Nursing theory and practice; biological, physical, social and medicinal sciences as related to nursing theory and practice; nursing techniques and procedures; nursing literature and current trends and developments in nursing.

ABILITY TO: Apply nursing theory and practices to planning and giving nursing care; recognize, interpret and evaluate symptoms of patient’s condition and take appropriate action; direct the work of nursing assistants; work effectively and cooperatively with patients, family members, medical staff and employees.
